Clinical Outcomes After Intensifying Antihypertensive Medication Regimens Among Older Adults at Hospital Discharge

Transient elevations of blood pressure are common in hospitalized older adults and frequently lead practitioners to prescribe more intensive antihypertensive regimens at hospital discharge than the patients were using before hospitalization.In national cohort study of 4056 propensity-matched older adults with hypertension who were hospitalized for noncardiac conditions, discharge with intensified antihypertensive regimens was associated with an increased risk of readmissions and serious adverse events within 30 days and was not associated with a reduction in cardiovascular events or blood pressure at 1 year.  The findings suggest that intensification of antihypertensives at hospital discharge is associated with short-term harms without long-term benefits and should generally be avoided in older adults hospitalized for noncardiac conditions.
