High-intensity interval training is effective and safe in patients with heart failure with preserved ejection fraction

Between July 2014 and January 2017 in a tertiary university hospital in southern Brazil was conducted a single-blinded randomized clinical trial on assessment of high-intensity interval training (HIIT, n=10) on functional capacity (peak oxygen consumption, VO2), quality of life score (MLHF) and LV diastolic function (E/e’)  compared with moderate continuous training (MCT, n=9) in patients with heart failure with preserved ejection fraction (mean age 60 ± 9 years, functional class II of the NYHA, EF > 50% and evidence of diastolic dysfunction, hypertension - 100% of patients, 63% women). Exercise training was performed 3 days per week for 3 months. After 12-week follow-up peak VO2 increased significantly in both groups over time, being higher in HIIT compared with MCT (22.7% vs 11.3%, respectively, p < 0.001). E/e’ ratio decreased significantly and QoL score improved over time in both groups. Multivariate analysis showed, that the increase in absolute peak VO2 was inversely correlated with reduction in E/e’ ratio (r = - 0.475, р = 0.04). There were not serious adverse events related to training.
